join 4ward Football

1. Voluntary Participation -Participation is voluntary and involves physical activity with some inherent risks.
2. Medical Responsibility - I confirm my child is medically fit. I will inform coaches in writing of any conditions/allergies.
3. Indemnity & Liability Waiver - I release and hold harmless 4Ward Football, its staff, and schools from any liability. I accept responsibility for medical expenses.
4. Emergency Care -I authorize staff to arrange medical treatment if I cannot be reached.
5. Code of Conduct - Respectful behavior is required. The program may suspend/exclude participants for misconduct.
6. Payment - I understand my obligation to pay fees on time and to give one month's written notice should i wish for my child not to continue with the program.

We respect your family’s privacy. All personal information is collected and processed in compliance with South Africa’s Protection of Personal Information Act (POPIA) and is used solely for registration, communication, and the safe participation of children in our programs.
